YUSA Pledge Sheet

 

Note: To use this pledge sheet, you must print it out first and then fill it in. You cannot fill in this sheet using your browser.  Please, please print clearly.

Select how much you wish to pledge:

(1) ___	$  5 per month (which equals  $60 per year) - One vote1
(2) ___	$10 per month (which equals $120 per year) - Two Votes1
(3) ___	$15 per month (which equals $180 per year) - Three Votes1
	In categories 1, 2, and 3 you will receive a program schedule via US Mail and on the internet; entrance to the Members Only Section which includes the Chat room.  Also, the listing of the shows that are running during the current two week period.
(4) ___	$20 per month (which equals $240 per year) - Four Votes1
(5) ___	Custom: $____ per month - You receive one vote for every $5 per month ($60 per year) you pledge.

¹ Since you will be helping to sustain the operation of the station, you will receive voting privileges. Many of the important issues concerning the daily operation of the station are determined by majority vote, among the persons who regularly support the station. You will receive "one vote" for each five dollars per month that you pledge.

 

 

You can pay for your pledge by credit card, if you wish. We accept Visa, MasterCard American Express, and Discover. No debit cards at this time.
